This test booklet, "Primary Examination: Form W for grades 2-3" for the New Stanford Achievement Test by Truman L. Kelley, Giles M. Ruch, and Lewis M. Terman was 5 pages long with a blank sixth page or back cover. The first page or front cover had a space for the students to write their name, age, grade, indicate whether they were a boy or girl, name of their school, and the date they took the test. It also had a space for the scores to be written, as well as teh average scores for each grade, educational age, and chronological age. Pages two through three were filled with test questions for test 1, Reading: Paragraph Meaning. Test 1 had 54 questions. The fourth page had the questions for Test 2 Reading: Word Meaning. There were 39 questions on this test. Page five had Test 3 Diction Exercise and dotted lines across the page. The test had not been filled out by any students.
