This is a blank report sent out by the International Film Bureau Inc. with their films, specifically the Grant Wood: His Life and Paintings Grant Wood's Life film set. This report could be filled out by the viewer and sent back to the International Film Bureau so that the company could best know how to market their product. The report had a space for the viewer to write the film title, subject area, which groups the viewer considered the film suitable for, comments on the film quality, comments on the film guide, and whether or not they would reccomend the film for purchase, with a space at the bottom for the viewer to put their information. The back is stamped with "PREVIEW REPORT Please Fill Out"
