This test booklet, "Primary Examination: Form W for grades 4-9" for the New Stanford Achievement Test by Truman L. Kelley, Giles M. Ruch, and Lewis M. Terman was 24 pages long. The first page or front cover had a space for the students to write their name, age, when their next birthday was, how old they would be after that birthday, grade, indicate whether they were a boy or girl, name of their school, and the date they took the test. It also had a space for the scores to be written, as well as the age equvalent and the grade equvalent. The second page had a chart with the test scores and how that fell along a national median as well as how the scores related to educational age, chronological age, and school grade. Test 1 Reading: Paragraph Meaning is from page 3 through page 5 and has 80 questions. Page 6 through page 7 is Test 2 Reading: Word Meaning and also had 80 questions. Test 3: Diction was pages 8 and 9 and consisted of dotted lines on the paper. Test 4: Language Usage was on pages 10 and 11 and had 74 questions. Test 5 Literature was 80 questions and on pages 12 and 13. Pages 14 and 15 are Test 6 History and Civics with 80 questions. Test 7 Geography is also 80 questions and on pages 16 and 17. Test 8, Physiology and Hygiene is 80 questions and is from page 18 to page 19. Pages 19 and 20 are for the 40 questions for the Test 9 Arithmetic Reasoning. The rest of the pages are for the 60 questions for Arithmetic Computation. This booklet was blank.
